Responsibilities:
Working under the direction of the Social Work Manager, the Substance Abuse Counselor provides services, assessment and program development for The PATH Center’s patients with alcohol or other substance use issues. This includes follow-up to ensure continuity of care as the patient uses outpatient, inpatient and community-based services. S/he works as a member of a multidisciplinary team that includes Attending Physicians, Nurses, Nutritionists, Case Managers and Social Workers.
Qualifications:
At least one year experience with HIV-positive patient population and CASAC certification required with BA/BSW/GED/High School diploma; CASAC preferred with MSW/LMSW/LCSW. Bilingual Spanish/English or Creole/English is preferred.
