THE ORGANIZATION
Heartland Alliance for Human Needs & Human Rights helps people living in poverty or danger improve their lives and realize their human rights. From a refugee fleeing violence and torture to a public housing resident trying to escape the cycle of poverty, Heartland Alliance helps people in the toughest of circumstances—who often have no place else to go. For more than 100 years, Heartland Alliance has provided hope, opportunity, and solutions—through both services and advocacy. Our work spans across four key areas, creating paths from crisis to stability and on to success: Housing, Health Care, Economic Security, and Legal Protection.
THE PROGRAM
The Homeless Student Support Initiative is designed to address the multiple needs of Chicago Public School students, and their families, in the Englewood community who are currently homeless or at risk of homelessness. Located in Englewood, the initiative will work with identified students at Harper, Hope, Henderson, Reed, Stagg, Wentworth, Wood, Robeson, and Parker schools. Services are home and school based, and the initiative is fully dedicated to working within and with the Englewood community.
THE POSITION
Senior Case Manager:
•Provides advanced advocacy, assessment, and case management services to individuals, families and groups.
•Conducts outreach and intake as assigned.
•Supervises program participants individually or in groups.
•Develops case management plans to assist program participants in addressing such problems as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem. •Counsels program participant individually, in family, or in groups and establishes appropriate service/treatment plans, reviews case progress and determines case closure according to program guidelines and standards.
•Refers program participants to community resources and other organizations.
•Helps program participant to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and program participant's part in creating them. •Provides case consultation as appropriate, and develops/implements special projects such as the development of informational materials, providing training for other staff in social work and treatment/service theory, and participating in assigned professional and community activities.
•Completes accurately and in a timely manner all necessary forms, case records and statistical reports. Submits such documentation to the supervisor/manager within the designated time lines.
•Transportation of program participants and/or Organizational property may be required. •Other duties may be assigned.
REQUIREMENTS
Education and/or Experience: High school diploma or equivalent and a minimum of seven years experience in case management, human services or related field, or equivalent combination of education and experience. A Bachelors degree may be required if mandated by funder and/or regulatory agency.
Additional Qualifications:
Other Qualifications: A valid driver’s license may be required.
