Program Description: The Family Life Management Center's Enhanced Teens program mission is to provide a higher and more intense level of services than that provided in the general prevention programs, meant to reduce the risk of teens being removed from their home
20 hours per week
Reports to: Clinical Supervisor
Basic Function:The Social Worker is responsible for providing quality comprehensive case management services within the context of the preventive services and reinvestment plan model.
DUTES & RESPONSIBILITIES:
• Provide clinical services to case load of 15 teens in line with the ACS approved reinvestment plan model.
• Conduct risk assessments.
• Assist as assigned in informational and health promotion workshops.
• Make home visits.
• Coordinate special projects.
• Conduct individual and group counseling.
• Other duties as assigned.
Additional Qualifications:
Bilingual (English/Spanish) is required.
LMSW or MSW preferred. Experience in case management, case planning, case work required and experience working with similar population. Experience with Administration for Children’s Services (ACS) Preventive Services Programs and/or Reinvestment Plan. Superior interpersonal skills. Thorough knowledge adolescent development and family systems. Good written and oral skills.
