Patient Navigator Program Manager – Make It Possible
NewYork-Presbyterian Hospital is a world-renowned medical institution dedicated to compassionate and patient-centered care. The hallmark of this commitment is our Patient Navigator Program, which helps individuals and families obtain post discharge care following an Emergency Department visit. We are seeking an outstanding leader to manage and grow this unique and worthwhile program at New York’s #1 hospital.
The Program Manager will be responsible for all aspects of development and implementation of the Emergency Department Patient Navigator Program in collaboration with Emergency Department and Social Work Services leadership. You must be highly motivated and organized and have a demonstrated ability to manage a large-scale project while working within a multidisciplinary team.
The Program Manager is required to:
• Recruit, train and manage program staff.
• Work with the Emergency Department and Social Work leadership to maintain productivity and resolve issues.
• Work with the IT department to develop and integrate tools for program management and evaluation.
• Prepare, analyze and present periodic and ad hoc reports pertaining to program outcomes.
• Create general program educational materials for ED and hospital staff.
• Create training manuals and guidelines for program evaluation.

Additional Qualifications:
To qualify, you must have a bachelor’s degree and a minimum of five years’ related work experience, including two years in management. Experience in healthcare, program development or grant management is ideal. Preferred candidates have a Masters degree, are bilingual (Spanish and English) and have excellent communication and data analysis skills. Strong proficiency with Microsoft Excel and Access software is desirable.
