Description:
HIV Youth Prevention Program
Position: Program Manager
Objective: To manage HIV prevention outreach and risk reduction activities to LGBTQ community provided by Project WOW!
RESPONSIBILITIES:
1. Delivering client services as described in the Cooperative Agreement awarded by the funding grant.
2. Assigning, coordinating and supervising the outreach activities of Prevention Outreach Workers (POWs) in the field. To help POWs be more successful in their efforts. (Emphasis will be placed on lifestyle, cultural and linguistic sensitivity)
3. Creating and maintaining confidential outreach contact / referral files with progress notes.
4. Ensure that 100% of the individuals enrolled in Project WOW! are referred and linked with general medical or HIV medical services and HIV psychosocial support services.
5. Ensuring that HIV individual and group prevention services are delivered to YMSMC who are enrolled in the project.
6. Ensure that all outreach and HIV prevention related activities are documented in accordance with the dictates of the funding grant.
7. Prepare and submit reports.
8. Delivering client services as described in the current grant contract.
9. Creating and maintaining confidential client counseling files with progress notes.
10. Preparing and delivering HIV prevention education presentations to YMSMC to inform participants and market the project’s services.
11. Document all services in accordance with the dictates of individual funding contracts.
12. Quality assurance including: charts, case conferences, administering and collecting client satisfaction surveys and outcomes-based internal evaluations .
13. Direct supervision of all Project WOW! employees.
14. Participate in manager and staff meetings with the Director of Social Services.
15. Provide input regarding program service planning and development.
16. Interact with other service providers to enhance coordination of services and referrals to the program.
17. Participate in governmental, professional and community committees, task forces, as assigned by the Director of Social Services.
18. Foster and preserve a strong team spirit among program staff and volunteers.
