Missão:
Village Care of New York (VCNY), in responding to the many individual needs of the elderly has developed a program complement that addresses the breadth and the depth of our aging community. This program complement of senior services are called the Urban Village. The Urban Village components provide a comprehensive range of nursing, medical, mental health, personal and social services that are designed to meet the needs of our program participants over an extended period of time.
Our mission and our programs reflect the experience we have gained from many
years of helping people face the physical, emotional and spiritual challenges associated with chronic illness, aging, disability and end of life. Guided by this experience, we work to afford all we serve the highest level of function and independence they can possibly attain. We involve client, patient and resident, their families and partners, in our treatment and care planning, enabling them to choose care that meets their expectations and need.
Our services are provided in both facility-based and community-based settings, with each program having a unique care delivery system and financial process. VCNY, with its continuum of care has created a delivery structure that integrates services vertically and horizontally, VCNY's Urban Village Programs provides a comprehensive range of care offerings that are designed to deliver quality care to a wide range of individuals.
