Description:

I. GENERAL SUMMARY OF DUTIES:

The Beacon House program provides temporary congregate shelter for 10 homeless families. The Case Management Supervisor provides case management services to these families, as well as providing clinical supervision and consultation to a Case Manager working with 15 families in CWU’s Rolling Stock Housing Program. Case management services include, but are not limited to: guest orientation, intake and assessment, case management, economic self-sufficiency planning, and general lifeskills. The mission of Crittenton Women’s Union, operationalized through program activities, is to assist individuals and families to preserve and maximize their life options and to attain and sustain self-sufficiency and economic independence.

III. ESSENTIAL DUTIES OF THE POSITION

1. CASE MANAGEMENT: Conducts needs assessment and intake in accordance with organizational and programmatic guidelines and standards; Develops individual service plans which define short- and long-term goals; Refers as needed for appropriate service provision and advocates on behalf of shelter guests accordingly; Provides counseling, crisis intervention, and prevention services as needed; Maintains regular contact with guests as specified in service plan and program requirements; Monitors guest needs and progress on an ongoing basis and updates service plans regularly; Maintains records and documents guest progress in accordance with program standards and contract outcome requirements; Participates in case conferences as needed around guest and service delivery issues.

2. CLINICAL SUPERVISION AND CONSULTATION: Provides regular clinical supervision and consultation to the Case Manager for the Rolling Stock Housing Program. Provides clinical support to Beacon House and Rolling Stock teams to effectively manage crisis situations and ensure that standardized reporting procedures are followed. Reviews intakes and assessments completed by the Rolling Stock Case Manager to ensure compliance with organizational, programmatic and practice guidelines and standards. Oversees the development of service plans. Convenes and facilitates case conferences as needed around client and service delivery issues.

3. SERVICE DELIVERY: Serves as primary liaison with Beacon House Property Manager and her staff. Provides and oversees case management services and related activities to ensure and support daily operations in accordance with program standards and outcome requirements. Works to ensure high quality in areas including, but not limited to: intake, assessment and service planning; ongoing support of families; liaison with and referral to external service organizations, as well as other programs at Crittenton Women’s Union; recordkeeping and reporting around client and program issues. Provides direct services, as needed, to families when necessary for program coverage or to manage crisis situations. Works to ensure effective program participation and adherence to program standards and outcome requirements. Plans and implements all service delivery activities as needed and in collaboration with staff and volunteers. Identifies, recommends and implements services in response to evolving guest and program needs. Collects and processes data and information to meet program requirements and to inform and support decision making. Maintains up-to-date documentation, both in case records and in the Efforts to Outcomes (ETO) database system. Participates in organization-wide efforts to monitor and improve quality of Case Management services. Prepares and disseminates routine and special reports as needed.

4. EXTERNAL RELATIONS: Serves as liaison to DHCD, DTA, and other community-based organizations to promote positive working relationships on behalf of families in shelter, promote visibility of the program, and ensure ongoing referrals and effective utilization of Crittenton Women’s Union’s services, and to meet program and contract enrollment and outcome goals. Participates in appropriate community and professional forums as a representative of Crittenton Women’s Union. Develops and strengthens the Program’s referral network.

5. MISCELLANEOUS: Performs other tasks as required.


IV. KNOWLEDGE, SKILLS & ABILITIES:
1. Intermediate knowledge of principles, procedures, techniques, trends, and literature of social work (e.g., mental health issues, human development, social policy, developmental disabilities, cultural factors, etc.) in order to meet social work responsibilities.

2. Basic knowledge of principles and procedures of human service supervision, including training and development of paraprofessional case management staff.

3. Intermediate knowledge of principles of mental health education in order to relay/clarify findings, when clinically indicated, to colleagues, community agencies (e.g., Department of Transitional Assistance, Department of Social Services, Social Security Administration, Department of Justice, etc.), interested persons, etc.

4. Basic knowledge of principles of community organization, case coordination, and advocacy to initiate/coordinate service delivery and follow-up services.

5. Intermediate knowledge of scope and activities of public and private health and welfare agencies in order to enhance the level of guest’s welfare and promote families’ long-term stability.

6. Intermediate knowledge of current standards in mental health, public health and public welfare, and Federal and State programs in these fields in order to provide advocacy and enhance guests functioning/access to identified community resources.

7. Intermediate knowledge of recognized treatment interventions such as risk assessment, family, group, and individual psychotherapies, psychosocial education, substance abuse interventions, crisis intervention, etc. in order to address identified service needs.

8. Advanced knowledge of National Association of Social Workers (NASW) Code of Ethics, Board of Behavioral Sciences (BBS) laws, rules and regulations pertaining to the delivery of clinical practice within the institution/parole mental health system in order to ensure professional competence.

V. EDUCATIONAL AND OTHER REQUIREMENTS:
A Master's degree in social work or equivalent discipline required. Two years of post-Master’s experience and Massachusetts professional licensure (LCSW or equivalent) preferred. Proficiency in Spanish helpful.

VI. PHYSICAL DEMANDS/WORKING CONDITIONS:
Requires prolonged sitting, some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment. Requires normal range of hearing and eye sight to record, prepare and communicate appropriate reports. Work may be stressful and fast paced at times. May require some evening and weekend hours.
