Description:
Employee Referral Bonus of $1200 and Sign On Bonus of $1200 paid in $600 increments after (a) successful completion of 90-Day Evaluation Period, and after (b) 6 months of continued employment.
SUMMARY OF POSITION:
Provides direct case work activities, timely intakes, assessments, advocacy and referrals. A minimum of 25 hours of direct casework is required per week.
ESSENTIAL FUNCTIONS:
Coordinates all assessment, develops case plans and referrals for treatment
Keeps daily time sheets and statistics
Discusses program and services with participants from intake through discharge
Establishes and maintains case records
Obtains all necessary records from other agencies involved with our COBRA clients
Meets with participants as needed for follow-up and recommendation
Teaches basic parenting and homemaking skills
Makes referrals for concrete services
Conducts home visits, case finding and outreach routinely in order to maintain caseload
Provides evening and weekend telephone coverage on a rotating basis
Performs other related and unrelated duties as directed by supervision

Additional Qualifications:
EDUCATION/EXPERIENCE:
AA in a related field required; B.S. in a related field preferred. Experience working with HIV-related illnesses, substance abuse, groups and children required. Experience with field work, home visits, case finding and outreach preferred. Ability to communicate clearly in both oral and written forms required. Ability to work effectively in teams required. Bi-lingual English/Spanish preferred. Drivers license and car a plus.
