Description:
Our Senior Services team has a rewarding opportunity for a Social Worker position at our offices in Fort Bend County. The individual that fills this position will be responsible for providing case management for seniors 60 years of age or older. These case management services will include: comprehensive assessments, service planning, case management, education, counseling, and referrals to appropriate community service providers. This individual will also be responsible for making client home visits, and transporting clients to medical appointments, grocery shopping, and/or pharmacy in agency vehicle. Ideal candidates will have a Masters degree in Social Work with an LMSW, and at least one year of experience working with seniors. Fluency in English and Spanish is required.
Additional Qualifications:
Candidates must have their LMSW.
