Description:
The DOT Outreach Worker will be responsible to provide the following:
* Ability to assess psychosocial barriers of treatment adherence in accordance with TAP guidelines.
* Utilizes harm reduction practices and principles.
* Knowledge of anti-retroviral medications, regimens, dosage indications, and side effects.
* Discuss and relay anti-retroviral medication information in appropriate language.
* Meets individually with patients following primary care provider referral to TAP program for intake into the TAP program, review provider treatment plan, assess treatment adherence physical barriers to adherence, behavioral barriers to treatment adherence, and determine ideal location for patient for health education visits, and document in accordance with TAP guidelines.
* Participates in initial and quartely case conferences with COMPASS program staff regarding patients enrolled in the TAP program and document in accordance with TAP guidelines.
* Collaborates with medical providers and with supervisor on treatment plans as indicated by TAP guidelines.
* Meets individually with patients to provide health education to enhance understanding of new treatment and promote adherence to treatment regiments, and education regarding other HIV related issues at patients' homes, or other offsite locations.
* Completes comprehensive initial quartely assessments of patients enrolled in the TAP and document following TAP guidelines.
* Manages pharmacy and medications for patients enrolled in the TAP program
* Develops linkage with other organizations involved in community health education and support services for patients
* Demonstrates the knowledge and skills necessary to provide care based on physical, psychosocial, educational, safety, and related criteria appropriate to the age of patients served in assigned area.
Additional Qualifications:
Bachelor's in Social Work, Psychology, Sociology, Behavioral Sciences, Health Education or relevant field preferred. Minimum working towards attaining Bachelor's degree required
The candidate must have the following:
* Experience and strong knowledge of and interest in HIV/AIDS clinical issues including role of alternative therapies.
* Experience working with diverse populations i.e., HIV, homeless, mentally ill, elderly, children, adolescents, etc.
* Excellent organizational skills.
* Experience working in a health care facility required.
* Bi-lingual spanish preferred.
