VOLUNTEER NUTRITIONIST

Lyon Martin Health Services is looking for a Volunteer Nutritionist to provide individual assessment, education, counseling and individualized nutritional plans to all Lyon-Martin patients (with specific attention to diabetic patients, HIV infected patients, those at high risk for cardiac disease, helping patients with weight loss, and helping patients with hypertension management). Lyon Martin works with many different populations, so is looking for a nutritionist who is comfortable working with patients of varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds, and economic status, especially queer, lesbian, bisexual and transgender patients, and patients who may be homeless, mentally ill, or substance using.

DUTIES AND RESPONSIBILITIES:
1. Provide individual assessment, education, counseling and individualized nutritional plans to all Lyon-Martin patients (with specific attention to diabetic patients, HIV infected patients, those at high risk for cardiac disease, helping patients with weight loss, and helping patients with hypertension management) according to pre-arranged schedules, documenting care promptly and following charting protocols.

2. Provide ongoing nutritional counseling/education to patients, in either a group or individualized format.

3. Work in conjunction with Lyon-Martin’s medical, nursing, social service, and patient services staff in the coordination of ongoing patient care.

4. Develop nutritional clinical protocols and standards for agency-wide implementation, as well as systems of communication between the referring provider and the nutritionist.

5. Provide in-service training on nutrition-related issues for Lyon-Martin staff.

6. Along with colleagues, assist in continuous quality improvement through on-going problem/needs identification and problem-solving.

7. Attend continuing professional education programs at the request or with the approval of supervisor, demonstrating participation in 30 hours continuing professional education (CPE) every 2 years.

8. Represent Lyon-Martin at and/or attend professional conferences, in-service training’s, and meetings at the request or with approval of supervisor.

9. Obtain and maintain licensure.

10. Complete all mandatory trainings on an annual basis, including but not limited to Confidentiality, Fire Safety, Incident Reporting, Child Abuse and Maltreatment and Domestic Violence, and Infection Control.

11. Comply with all aspects of Lyon-Martin Policies and Procedures applicable to Medical Staff members as well as all Lyon-Martin agreements with insurers and other entities that contain provisions applicable to or affecting the duties and responsibilities of Medical Staff; and all applicable statutes, regulations, governmental agency guidelines, and non-governmental oversight body requirements that place requirements on either Lyon-Martin or Medical and Dental Staff

12. Perform other duties as assigned by supervisor to maintain a high level of patient-centered care.

EDUCATION AND SKILLS:
1. Registered Dietician
2. Ability to work cooperatively with others and demonstrate good judgment in unusual and emergent situations; ability to manage and de-escalate crisis situations.
3. Ability to work independently and mange multiple tasks simultaneously.
4. Flexibility in working in a small, fast-paced non-profit clinic.
5. Bilingual Spanish/English preferred.

EXPERIENCE:
1. A minimum of 2 years experience, preferably in a medical setting
2. Experience working with homeless, mentally ill, and substance using patients.
3. Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds and economic status.
4. Experience working with lesbian, bisexual and transgender patients.
