Pine Habilitation and Supported Employement, Inc. (PHASE) is a private, non-profit organization that provides an array of employment-based and additional supportive services to adults with a diagnosis of a developmental disability, traumatic brain injury, or severe and persistent mental illness.
PHASE develops services to meet the individual needs of each Program Participant, including student transitions, community employment, facility-based supportive services, and facility-based living skills training.
We are located in East Central, Minnesota, and operate out of three main offices: PHASE North (Sandstone, MN), PHASE South (Pine City, MN) and PHASE Recycling (Sandstone, MN).
