We are looking for experiences in:
Physical Therapist:Minimum requirements: Current NYS PT license and registration, a Bachelor's Degree, and at least two (2) years full time professional experience as a Physical Therapist in a CHHA home care setting, as well as one (1) year full time professional experience as a rehab therapist in an MLTC home care setting.
Enrollment Nurse: Village Care Plus, our licensed home care agency, is seeking experienced professional Registered Nurses (NYS RN) –; to provide care planning, coordination and to conduct and evaluate on- going assessments of health, environmental, and psychosocial status of clients' through home visits. Minimum Requirements: Current NYS Registered Nurse (RN) license and registration, with a Bachelors or Associate Degree in Nursing preferred, with one (1) year professional job experiences as a RN in home care.Bilingual in Spanish is a MUST
