Description:
Proyecto Salud is recruiting for a 0.5 FTE Health Educator to provide medical nutrition therapy to patients with chronic conditions that are poorly controlled, in addition to coordinate the clinic health education component. Some of the goals of Proyecto's education program are to promote, maintain, and to improve patients and community health outcomes by assisting individuals to adopt healthier behaviors.
Proyecto Salud Health Educator should be able to develop operational plans and policies necessary to achieve the health education program goals.
Requirements for the Position:
- Minimum two years of experience on Adult health education.
- Bachelor's Degree preferred.
- Certified Diabetes Educator a Plus.
- Strong Knowledge in Nutrition for chronic conditions such as Diabetes, HTN, Dyslipidemias, Obesity.
- Must be Bilingual in English and Spanish.
- Strong verbal and writing communication skills.
- Solid computers skills.
- Experience with electronic medical records EMR.
- Comfortable working on a fast pace/team work environment.
Salary is commensurate with experience.
