The Housing 1000 Case Manager will work throughout Santa Clara County, identifying our most chronically homeless individuals from the Santa Clara County Housing 1000 list. The Housing 1000 Case Manager will work to get these clients housed and provide ongoing case management services.
The case manager will engage in regular and ongoing case conferencing consisting of meeting with Housing 1000 Project Coordinator and other Intensive Case Managers to coordinate client services, identify gaps, track outcomes and update case management plans. The case manager shall participate in the Housing 1000 Care Coordination project and share data and information regarding outcomes and housing placements with Destination: Home and the Housing 1000 Leadership Team.
PRIMARY RESPONSIBILITIES:
- Outreach Case Management: Identify target population and perform screening assessments and ongoing case management to assist participants to meet their goals. Provide information, referrals and crisis intervention as needed. Keep case files up-to-date, thorough, and accurate. Assist in compiling program statistics.
- Housing Case Management: Provide case management services to formerly homeless clients residing in scattered site transitional and permanent supportive housing. An emphasis will be placed on increasing life skills for self-sufficiency.
- Job Development: Assess client's work readiness, make referrals, and provide job search skills training. Create and maintain job listings. Develop contacts with potential employers. Maintain professional relationships with referral agencies.
- Housing Readiness: Work with homeless clients to transition them quickly to permanent housing. In some cases this will require referrals to shelter while individuals conduct housing searches. Provide housing counseling. Create and maintain working relationships with local landlords and leasing agencies. Provide follow-up case management as needed to housed clients.
QUALIFICATIONS:
- Bachelor's degree in a related field preferred
- Strong skills in intensive case management and report writing
- Excellent interpersonal skills and ability to interact effectively with other agencies
- Prior experience with homeless outreach and mental illness a plus
- Must understand and posses a willingness to learn the issues faced by our most chronic homes individuals
- Automobile, valid driver's license and clean driving record
- Proficient in Microsoft Office; knowledge of HMIS a plus
