Emergency/Transitional Housing Program provides transitional housing services to New York City clients living with HIV/AIDS. The program has eleven two-bedroom apartments in the community, each of which is shared by two clients. In addition, the program provides 7 transitional community-based apartments for families. The program's goal is to prepare clients to live independently. Participation in the program is generally limited to one year. Services include case management, supportive counseling, treatment adherence, independent living skills and nutritional programs. The program serves approximately 50 clients per year.
The person filling this position is expected, under general supervision,to:(1) aid individuals requiring social service assistance;(2)formulate service plans and goals, and assist clients to implement service plans;(3) help clients to prepare for permanent housing, including locating safe affordable units; (4)locate and make use of appropriate community resources for clients; and(5) provide general oversight of clients and operations in the facility; etc.
The minimum qualification for this position is a Bachelor's degree (e. g., B.A., B.S.W.) and two years of applicable experience and/or equivalent experience.
