Outpatient mental health center in Brookline seeks full time case manager to work half time with adults with mental and physical illness and half time with families in the Brookline and metro Boston communities. Primary responsibilites include home visiting to help adults and families manage day to day living taks including but not limited to: assistance with attending medical appointments, accessing benefits, organizing the home, caring for medical ailments, communicating with other providers, etc. Data management and administrative tasks required to support program research and evaluation. Weekly individual and group supervision provided. We are looking for at least a year's committment to this position. Minimum of B.A. with 1-2 years of experience in social service field. Car Required.
