TITLE: Community Habilitation Instructor for Long Island, NY
QUALIFICATIONS:
High School or general equivalency diploma.
Experience working with individuals with developmental disabilities and/or physical disabilities.
Speak, read, and write English
RESPONSIBILITIES:
- Work directly with an assigned participant(s) on the habilitative activities specified in the Community Habilitation plan.
- Travel into community as required by the Community Habilitation plan.
- Have a working phone line (cell or land line) for work relative communication.
- Teach the participant in skill development through implementation of the Community Habilitation plan.
- Assist in the planning and/or review of the Community Habilitation outcomes along with the family, participant, family advisor and program coordinator.
- Write comprehensive progress notes.
- Submit progress notes and time sheets on or by Monday NOON weekly.
- Complete and submit Data Sheets for the previous month on or by Monday NOON of the first Monday of each month.
- Follow all specified procedures, especially those regarding emergencies and incident reporting. Report incidents immediately.
- Communicate with the HCBS Coordinator, or their supervisor any concerns or problems with service delivery.
- Undergo a criminal history/fingerprinting background check.
- Undergo a child abuse registry check
- Good attendance and punctuality a must.
