Job Title: Case Manager (Scattered Site)

FLSA Status: Exempt

Reports To: Department Director

DATE: 7/2/13

POSITION SUMMARY:

Under the direction of the Department Director, the Case Manager is responsible for providing primarily home-based case management services for families experiencing homelessness who are rehoused in apartments scattered throughout the District. The primary goal is to assist families in addressing the barriers to maintaining permanent housing. Functions performed include assessment, follow-up, review, plan development, research, and problem solving with other Case Managers. Work is accomplished by advising, and motivating program participants to accomplish points in their Family Service Plan.

ESSENTIAL FUNCTIONS:

  • Coordinate with THC Housing Team to locate housing options for family
  • Develop and update Family Services Plans (FSPs) with program participants, assisting them to set and prioritize short and long-range goals
  • Support families in performing the tasks required to maintain housing stability, including on-time payment of rent and household maintenance
  • Meet with program participants to help them set and take manageable, achievable steps toward goal achievement, empowerment and self-reliance, providing encouragement, support, information, referrals and resources
  • Provide referrals as needed to THC supportive services (employment, mental health, substance abuse, life skills, and youth services) and work collaboratively as a team to advance the goals of the client
  • Participate actively in case consultations with other caseworkers and program staff, seeking and providing advice and suggestions for effective case management in general and in individual situations
  • Maintain participant records in Homeless Management Information System (HMIS) database and other case files as needed, recording narratives and other required information regarding status and progress. Maintain and ensures confidentiality of participant information.
  • Capture daily and monthly statistics and other required reports as directed
  • Maintain effective working relationships with other agencies to which (and from which) participants may be referred, regularly contributing to the resource directory/database
  • Participate in THC special events, helping to share information about THC and working, as needed, to ensure event success
  • Attend staff meetings and other meetings as required by supervisor

Knowledge and Skills Required

  • Bachelor's degree in social work or related field plus 1 year of experience in social/human services field; OR six years of experience in the social/human services field demonstrating progressive responsibility
  • Social workers must obtain licensure within 90 days of hiring
  • Excellent interpersonal and conflict resolution skills
  • Excellent listening and analytical skills
  • Strong oral and written communication skills
  • Team player
  • Basic computer skills (Microsoft Office, internet, email) and previous experience with databases
  • Knowledge of community resources or the ability to become knowledgeable
  • Strong initiative and ability to be flexible with a new program
  • Strong organizational skills
  • Valid driver's license and vehicle required, as well as proof of auto insurance

Knowledge and Skills Desired

In addition to the qualities listed above:

  • Experience working with families, homelessness, and/or substance abuse strongly preferred

Work Environment/Physical Effort

  • Must be willing to travel to different locations throughout Washington DC
  • Regular hours are 9am-5pm with occasional evening and weekend hours

SALARY AND BENEFITS

$40,000 - $43,000 annual salary, depending on experience. THC offers a competitive benefits package including health insurance and a retirement plan, paid vacation, and sick leave.
