Job description

Job Requirements

  • Provides primary care, routine and acute care for scheduled and walk-in appointments
  • Performs routine diagnostic and therapeutic procedures according to accepted practice standards
  • Provides clinical supervision to volunteer physicians, mid-level providers (PA and NP) and other clinical staff; signs off on clinic patient records
  • Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, asthma, bronchitis, diabetes, lipid disorders, and hypertension
  • Generates appropriate referrals to ancillary and specialty services including medical, mental health and social based services
  • Demonstrates proficiency with electronic medical records; including documentation, e-prescribing and coding
  • Maintains documentation and e-prescribing practices according to standards of the agency and regulatory agencies including but not limited to history and physical, social history and literacy level as well as care management
  • Completes and locks progress notes within 48 hours of client encounter. Ensures timely prescribing, orders, addressing labs, interpreting of test results, addressing telephone encounters, ordering and reviewing of specialty referrals and consultation reports
  • Provides health education and counseling during visits
  • Provides on-call telephone coverage if requested
  • Participates in CQI activities
  • Attends community based activities, including advocacy efforts for immigration reform, to eliminate homelessness, and address disparities in healthcare as designated by Executive Director
  • Other duties as assigned by the Executive Director

Additional Qualifications

In addition to the above, the ideal candidate will possess most or all of the following:

  • Experience working with hard to reach populations, including the homeless, immigrants, HIV infected, substance users and/or the mentally ill
  • Desire and ability to work as part of an interdisciplinary team dedicated to the delivery of high quality care
  • Rockaways resident, or individual with experience living and/or working in the Rockaways
  • The spirit and desire to practice in a community based, interdisciplinary, mission driven system of care
  • Willingness to work off hours on special projects as reasonably dictated by Executive Director
