Job description

POSITION RESPONSIBILITY: Hours- Sun-Thurs 1st shift (6:45AM to 3:15PM)

Responsible for implementing the clients' active treatment plans and ensuring clients receive appropriate care. Implement and oversee daily activities andcare of clients. Advocate for the client's rights, needs, and desires. Work to foster and enhance
each individual's independence. Supervise direct care staff.

SPECIFIC DUTIES & TASKS:

1. Implement formal and informal active treatment activities and documentation as assigned by the QMRP and SSCM.
2. Supervise the implementation of clients' daily schedules by DSP staff including positioning and activities.
3. Work in conjunction with on-floor shift Nurse to supervise assigned DSP staff.
4. Participate in annual assessment process in preparation for the annual Habilitation Planning Meeting for clients as assigned by the QMRP.
5. Maintain daily active treatment/program progress notes for assigned clients.
6. Ensure each client is provided with ongoing engagement and activities throughout the day.
7. Ensure appropriate level of supervision is maintained for all clients.
8. Ensure a safe and healthy environment is maintained for clients and staff.
9. Monitor DSP staff and Nursing staff regarding behavior protocols, meal protocols and nutritional management, positioning and physical management, active treatment programs, self-medication programs, direct care, sensory and engagement activities, etc.
10. Monitor meal time and assist DSP staff and Nurse in reporting meal card discrepancies to kitchen.
11. Monitor use and application of body braces and splints in conjunction with nurse.
12. Provide competency based training, including modeling and feedback, to DSP staff and Nursing staff as needed during training and monitoring activities.
13. Complete program, training, and monitoring documentation as assigned by QMRP.
14. Ensure that cleanliness and integrity of therapeutic and programming equipment is maintained.
15. Ensure that needed repairs of therapeutic equipment and programming equipment are reported to the QMRP.
16. Train new staff using the Extended Orientation process. Monitor new staff and provide competency based training, including modeling and feedback, to new staff.
17. Initiate and coordinate community activities for clients.
18. Assist clients to develop natural community supports.
19. Monitor and assign housekeeping chores to DSP staff as needed to ensure cleanliness and organization of clients' environment and belongings.
20. Monitor and assign stocking duties to DSP staff as needed to ensure client personal and facility supplies are adequate.
21. Communicate with SSCM regarding client hygiene/clothing/personal belonging needs and ensure completion of documentation assigned by SSCM.
22. Maintain a positive professional relationship with co-workers; support co-workers and other departments via effective communication.
23. Coordinate with shift nurse to schedule staff breaks.
24. Monitor and provide feedback to DSP staff for late arrivals, lunch breaks, leave early, and compliance to policies/procedures during the shift.
25. Provide written and/or verbal clarification to staff who fail to follow "active treatment plans/protocols" developed by the IDT.
26. Complete monthly Performance Evaluations for DSP staff.
27. Complete other documentation as assigned.
28. Communicate professionally and effectively with consultants including Physical Therapy Assistant, Occupational Therapist, Speech Therapist, Physical Therapist, Dietician, Psychologist.
29. Direct all consultant changes and recommendations to QMRP. Ensure QMRP is aware of any communication from consultants.
30. Work in conjunction with RPTA regarding positioning, transfers, troubleshooting PT issues, etc.
31. Provide for a continuum of care and treatment for clients by communicating and sharing information with Program Specialists, DSP staff, and Nurses on incoming and outgoing shifts.
32. Attend and participate in staff trainings and meetings.
33. Operate company vehicles as requested by supervisor.
34. Follow all Quest, Inc. and Laurel Hill Cluster Policies and Procedures.
35. Maintain a family home atmosphere, normal routine, positive environment for clients; ensure clients' dignity and rights
36. Comply with safety regulations and standards.
37. Complete other duties as assigned.

JOB REQUIREMENTS. Requirements are representative of minimum levels of knowledge, skills, and/or abilities. To perform this job successfully, the incumbent will possess the abilities or aptitudes to perform duties proficiently.
1. ABILITY TO: Handle multiple projects simultaneously
2. ABILITY TO: Use a personal computer, mouse, and keyboard (basic typing)
3. ABILITY TO: Supervise staff effectively: lead, direct, teach, delegate
4. ABILITY TO: Basic math skills including: add, subtract, multiply, and divide whole numbers and decimals; calculate percentages.
5. ABILITY TO: Write and speak fluently in English using acceptable grammar and punctuation.
6. ABILITY TO: Lift 50lbs repeatedly. Bend, kneel, and crouch repeatedly.
7. ABILITY TO: Understand and follow multiple-step written and verbal instructions in English without supervision.
8. ABILITY TO: Act as a positive role-model for staff
9. ABILITY TO: Clearly communicate expectations to staff
10 ABILITY TO: Recognize staff accomplishment and motivate staff to perform

QUALIFICATIONS:
Education/Training:
High School diploma or equivalent (GED) required.
AA/AS Degree Preferred.
Basic computer skills. Knowledge of MS Word and Excel
Minimum 1 year experience working with people with Developmental Disabilities required. 2 years supervisory experience preferred.
Valid Florida driver's license, good driving record.
