For almost 200 years, Episcopal Social Services (ESS), accredited by the Council on Accreditation, has helped meet the pressing needs of the vast numbers New York City's disadvantaged. ESS has remained flexible in responding to the changing needs of the disadvantaged with an unchanging mission to help children, families and other individuals become independent and self‐sufficient, and to make the most of their opportunities and potential. Today ESS, through its multi‐site programs and services, meaningfully touches over 5,000 lives every year throughout all of New York City, with an emphasis on the South Bronx and Manhattan. ESS's non-sectarian work focuses on strengthening children, families and adults through programs in foster care and adoption, early childhood education, after‐school initiatives, and group homes for developmentally disabled adults.

Our organization is seeking to hire a Medicaid Services Coordinator. The ideal candidate will be able to effectively:

  • Oversee and coordinating clinical issues.
  • Screen candidates for placement and developing a waiting list.
  • Develop and coordinate the Preliminary Individualized Service Plan (PISP).
  • Develop, oversee implementation and maintain of the Individualized Service Plan (ISP) based on the individual's personal goals, choices and outcomes.
  • Review, monitor and revise the ISP to reflect the changing needs, choices and preferences of the individual, minimally on a semi-annual basis and more often as requested by the individual, family and/or advocate. Forward copies of ISP to all relevant parties in a timely fashion.
  • Ensuring that all services described in the ISP are delivered in a timely and appropriate manner. Ensure quality service delivery, as well as, regulatory compliance.
  • Act as liaison between outside service providers and the individual. Encourage integration, independence, productivity and individual satisfaction in all community activities.
  • Document all contacts and service coordination activities. Prepare monthly service coordinator notes. Ensure documentation is up-to-date, complete, comprehensive, and filed in the individual's record at all times.
  • Assess each individual's needs via face-to face monthly contact and gather information from collateral sources.
  • Assist with quality improvement chart reviews.
  • Participate in the development of appropriate in-service staff training.
  • Maintain an ongoing caseload as assigned by supervisor.


  • Bachelor's Degree in Human Services or related field.
  • Two (2) years of experience working with people with a developmental disability.
  • Participation in OPWDD CORE training curriculum is required within first 3 months of employment.
  • Knowledge of community resources.
  • Must be familiar with ICF/MR regulations and waiver requirements.
  • Ability to relate and interact positively with residents, staff and other providers of service.
  • Ability to coordinate. Good writing skills.
  • Valid NYS State Drivers License preferred.
