We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.

The Doe Fund is currently seeking several high-energy, self motivated Case Managers to join our growing team. We currently have positions open in both our traditional Ready, Willing & Able program and also in our Veteran's program. These positions will be based out of our Porter Ave. Brooklyn, New York location.

RESPONSIBILITIES:

  • Manage a caseload of clients, monitor their progress toward meeting goals established in their service plans and their participation in required RWA or Veterans program components.
  • Research and coordinate access to outside referral sources for Mental Health, Substance Recovery, Benefits, Entitlements, and Primary Care services.
  • Collaboratively with clients, provide assistance in goal setting and self-development; work as a team with other staff to achieve program wide participation, graduation and retention goals among others.
  • Organize and facilitate groups on various topics.
  • Maintain current, accurate, complete and timely documentation of trainee progress in charts and databases (TDF's Client Tracking Database and DHS's CARES).
  • Provide crisis intervention to clients in order to resolve any issues or concerns that may arise.
  • Advocate for appropriate client housing and cultivate housing contacts i.e. landlords and real estate offices when necessary for Veteran clients.
  • Interact with Department of Homeless Services and Veteran Affairs representatives as necessary.

QUALIFICATIONS:

  • Bachelor degree
  • At least 2+ years of experience in Human Services/Case Management
  • Experience with Veterans a plus
  • CASAC preferred
  • Strong interpersonal and written/verbal communication skills required
  • Working knowledge of MS, Word, Excel, PowerPoint and MS Outlook
