Provide health and nursing services to Adult Day Health Center clients.
This position requires advanced knowledge of principles and practices within a professional field or recognized body of formal knowledge.
Knowledge of HIV disease Prior work experience with substance abuse and mental illness
Monitor the health status of each client and conduct physical assessments as necessary
Provide and document direct nursing treatment based on assessment, care planning and approval by clients PCP
Conduct 30/90 day assessments which includes medication adherence, pain assessment, education about lab work, discussion about high risk behavior and general HIV/AIDS awareness
Complete RAI and initial nursing assessments for newly enrolled clients within 30 days of admission
Complete medical care plans quarterly and prn and attend clinical/care plan meetings for individual clients
Execute medical regimens authorized by PCP (ie. Injections, med box filling, calling in refills to pharmacy, etc)
Ensure medical charts are in good order
This position requires a minimum formal education of Bachelor's Degree and a minimum of 1 year job-related experience working in Health Care facility or HIV Population
