Lenox Hill Neighborhood House is looking for a Geriatric Case Manager to join our Case Management team, which provides comprehensive case management services to thousands of frail and homebound older adults on Manhattan's East Side. This is an exciting opportunity to help serve the ever-growing population of older adults on the East Side and to be a part of a supportive working environment that links case managers, social workers, lawyers, and others providers together in our efforts to support the most frail members of our community.
Lenox Hill Neighborhood House, widely recognized as one of New York's premier human services providers, is a 120-year-old settlement house that provides an extensive array of effective and integrated services--social, educational, legal, health, housing, mental health, nutritional and fitness--which significantly improve the lives of 20,000 people in need each year, ages 3 to 103, on the East Side of Manhattan. Our constituents include older adults, homeless and formerly homeless adults, children and families, recent immigrants, disabled persons, adult learners and thousands more. Working as part of our Case Management team, the Geriatric Case Manager will assess the needs of frail, homebound adults; develop a care plan to help meet these needs; and effectively implement the care plan services. These services will include: providing information about and linking clients to appropriate services and programs, applying for government benefits and other financial programs to increase economic security, advocating for the clients' medical, mental health, housing-related and other needs, assisting with bill paying and generally providing comprehensive services designed to enable homebound older adults to age in place with improved outcomes and quality of life. The Geriatric Case Manager is also expected to timely maintain and input accurate case records into our case management database.
QUALIFICATIONS: The successful applicant must possess excellent interpersonal skills and strong computer skills, as well as experience in the field of human services, particularly related to older adults. BA or BSW accepted but MSW preferred. Competitive salary with a comprehensive benefits package available.
