The Housing Coordinator ensures the quality of care for program participants who are living with HIV/AIDS. This individual will help to prepare program participants for permanent housing and assist in their placement in permanent housing.
- Develop a "Re- Housing" Plan with each consumer within thirty days entry into transitional housing program. - Conduct monthly Housing Workshop to educate consumers on their options and develop skills for obtaining and maintaining permanent housing. - Conduct individual and group Independent Living Skills Training Workshops. - Liaison to community housing providers to establish network of commercial, private, subsidized, and government-sponsored permanent housing options for placement of consumers. - Assist consumers with obtaining all necessary paperwork and documentation to be eligible for housing. - Assist consumers with obtaining and maintaining public benefits to be eligible for housing. - Accompany consumers on housing inspections and interviews, as appropriate. - Document all services in compliance with agency standards. - Work as a member of the social service team in the development of on-site and off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration. - Liaison to community based services and groups including civic, social service government, and business organizations. - Participate in ongoing individual supervision, team meetings, case conferences, division meetings and trainings. - Adhere to on call beeper system to facilitate crisis intervention, emergencies and other situations that require immediate action. - Participate in the Services for the Underserved Quality Assurance/ Quality Improvement activities as appropriate to role. - Performs other duties related to the program as defined by the Clinical Supervisor. - Reports to the Site Director in the absence of the Clinical Supervisor. - Working knowledge of various computer programs including e-mail, wide area networks, Microsoft Word (latest version), Excel, Powerpoint and Windows Operating System. - Evening and weekend hours may be required.
- Bachelor's Degree in Human Services field required - A minimum of three years work experience in social services, two of which working with individuals with HIV/AIDS, substance abuse issues and a history of homelessness strongly preferred. - Experience in residential setting strongly preferred. - Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred. - Bilingual skills preferred.
