NHTD Service Coordinator (Chelsea)
SKIP is a nonprofit case management organization that helps medically and
developmentally disabled individuals attain and retain services while staying in their
home environment
NHTD Service Coordinator position:
Requirement;
Bachelors or * higher degree in social work, psychology, gerontology or other degree with major in Human Service related field:
Minimum of 2 years experience as Service Coordinator, providing information, linkages and referrals regarding community based services for individuals and or seniors with disabilities.
Knowledge of Medicaid and Medicare services and entitlements a plus.
- For those who have master's degrees, only one year experience is required.
All applicants must be an experienced driver with a valid driver's license.
