Five (5) full-time positions available in Bellingham, Puyallup, Tacoma, White Center, and Vancouver respectively.
MSW, MA, MS in counseling or similar human service related field (MSW preferred) or RN with social service experience. Full licensure with the State of Washington is required for this position (associate licensure does not apply).
The Integration Specialist II functions as a core member of the Health Home team, a collaborative care team that involves the patient, the patient's primary care provider, medical staff, and behavioral health staff. S/he is responsible for providing assessments and screenings, brief intervention and/or brief treatment for those with co-morbid conditions that can include substance use disorders and behavioral health concerns that compound self-care of medical diagnoses. The Integration Specialist II is responsible for timely and effective referrals to and communication with both internal Sea Mar services and community-based resources, and for facilitating high levels of continuity of care.
Bi-lingual Spanish/English preferred, but is not required.
