The Clinical Case Manager is responsible for providing supportive case management services to the Scatter Site Return clients including assistance with obtaining public benefits, monitoring clients' rent and utility payments, developing independent living skills, socialization skills, facilitating groups and recreational activities.

RESPONSIBILITIES:

  • Maintain a caseload of clients with active substance abuse issues, conduct home visits and provide supportive individual counseling to these individuals.
  • Monitor and assist clients with rent and utility payments.
  • Assist clients in accessing and maintaining benefits.
  • Conduct apartment inspections and address any maintenance issues with property management staff to assure safety of the apartments.
  • Maintain client records and complete a variety of charting documentation as scheduled.
  • Develop and implement individual service plans and assist clients with achieving their goals geared toward program graduation.
  • Develop and facilitate support groups and recreational activities for clients of the Scatter Site Return housing program.
  • Educate clients about approaches to managing substance use.
  • Maintain a liaison with community services including detoxification/drug rehabilitation, substance abuse treatment programs, and mental health programs.
  • Participate in weekly individual supervision and staff meetings.
  • Perform other duties as assigned by the supervisor.

QUALIFICATIONS:

  • Bachelor's Degree in Social Work or related field. A Master's degree is preferred.
  • Experience working with homeless population.
  • Knowledge of psychiatric diagnoses, substance abuse and mental health issues.
  • Strong organizational, writing and communication skills.
  • Fluency in Spanish is required.
