Maternal and Child Health Consortium of Chester County, (MCHC), a leading advocacy and service agency for women, children and families in Chester County, Pennsylvania is seeking a Information and Referral Specialist. This position reports to the Senior Director of Healthy Start and Family Benefits Programs of MCHC.
The Information and Referral Specialist responds to requests for social and human services in Coatesville, PA.
Responsibilities Include:
- Conduct assessment of the person requesting services and make appropriate referrals.
- Provide relevant information on, and referral to, community resources providing or making available the needed services.
- Follow-up on referrals to determine outcomes, as appropriate.
- Provide Information and Referral (I&R) services in accordance with the rules established by the Human Services Development Fund (HSDF).
- Utilize database to provide I&R services.
- Ensure accommodations are made to meet the needs of the non-English speaking residents needing I&R services.
- Submit monthly reports and outcome data to the Department of Human Services (DHS).
- Attend events and organizational-wide staff meetings.
- Other duties may be assigned.
Qualification Requirements Include:
- Fully bilingual in both verbal and written Spanish and English.
- Fully bilingual in both verbal and written Spanish and English.
- Excellent verbal and written communication skills.
- Track record of being self motivated, with ability to work independently to achieve assigned objectives.
- Familiar with the Coatesville area.
- Ability to communicate effectively with people from diverse communities.
- Intermediate computer skills (e.g. Keyboarding, Windows, Internet Explorer) including ability to enter and track participant data.
- Capability to work flexible hours (some evenings and weekends) and to occasionally travel overnight to attend training programs.
- No criminal or child abuse history as determined by criminal history and child abuse screenings including, but not limited to, FBI clearance.
- Bachelor's degree in Public Health, Health Education, Social Work or related field.
- Associate's degree and two years of relevant experience may be substituted.
- Prior case management experience.
