Job description


Under the direction of the Case Planning Services Manager, the Case Planner will meet individually with participants to complete intake, assessment, goal setting and case management.


  • Provide individual and family case planning, and needs assessments.
  • Research and develop community resources and support services to meet the needs of assigned clients.
  • Maintain on-going contact with clients, making home visits, monitor living situation, insuring client's involvement in support groups, and complete progress reports on a quarterly basis.
  • Ensure that participant discharge planning activities and documentation are completed within 48 hours of discharge.
  • Collaborate with Shelter/TIL team members to insure accomplishment goals.
  • Provide direct care and supervision of shelter residents.
  • Ensure paperwork is maintained in accordance with State of Michigan Child Caring Organization licensing requirements, as well as AFG policies and procedures.
  • Complete intake data entry in a timely manner.
  • Ability to handle crisis, maintain confidentiality, and tolerate stress professionally.


  • Bachelor's degree in Social Work, Criminal Justice, Sociology, Psychology, or Guidance and Counseling is required.
  • Minimum of 2 years experience working with high risk adolescents, case planning, individual/family counseling, case management, assessment, intervention, documentation and culturally diverse populations is preferred.
  • Interpersonal skills sufficient to communicate with participants, public and staff.
  • Composition skills sufficient to prepare required reports.
  • Meet all licensing requirements: TB test, DHS clearance, background check, etc. Valid Michigan Driver's license, excellent driving record and access to personal vehicle.


  • Ability to safely and successfully perform the essential functions consistent with ADA, FMLA and other federal, state, and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Position involves the ability to work flexible schedule including weekends, evenings and early morning.
  • Work involves the ability to push and lift program donations (i.e., electronic equipment, office furniture and accessories) weighing up to 50 pounds.
