Family Nurse Practitioner (APN)

This position is accountable for promoting excellence in Clinical Nursing practice by providing expertise in the following areas:

  • Advanced clinical practice to enhance the quality of patient/client care
  • Education of the patient, family, staff and the community
  • Development of health promotion programs
  • Assisting with data collection

The Advanced Nurse Practitioner conducts comprehensive health assessments, demonstrates a high level of autonomy and expert skill in the diagnosis of complex health responses of individuals, families and community to actual or potential health problems. The Advanced Nurse Practitioner formulates clinical decisions to manage and promote wellness. The Advanced Nurse Practitioner integrates education, research, management and leadership in a clinical role and functions in a collegial relationship with other health care professionals to insure continuity of healthcare:

  1. Develops, monitors and alters a comprehensive health care treatment program through:
  2. * Formulating a comprehensive data base by physical and psychological health history, physical examination, and evaluation of laboratory diagnostic testing, as indicated.
  3. * Discriminating between normal and abnormal findings in planning treatment interventions.
  4. * Discussing significant abnormal findings with other health care professionals.
  5. * Synthesizing complex clinical data in order to formulate implement and evaluate a nursing management plan for individuals.
  6. * Modeling interdisciplinary collaboration with other health care professionals in the school to ensure continuity of care and optimal patient/client outcomes.
  7. * Instructing patients in the acquisition of information related to health and wellness.
  8. * Initiating and implementing appropriate consultations and referrals.
  9. Maintains and expands current knowledge base regarding advanced practice nursing.
  10. Develops specific health programs for patients.
  11. Assists in collection of clinical data.
  12. Demonstrates clinical practice that reflects research based foundation.
  13. Disseminates research findings.
  14. Assists in collection of clinical data, keeping appropriate databases updated in a timely manner.
  15. Maintains appropriate charting and encounter documentation.
  16. Prepares monthly medication orders.

Education & Qualification Requirements:

  • Master of Science Degree in Nursing from an accredited School of Nursing.
  • Minimum 1-3 years' experience in ambulatory care.

Certifications/Licensures:

  • Valid Indiana NP license.
  • National Certification as an Advanced Practice Nurse.
  • Maintain appropriate BLS certification.
  • Maintain a current CSR and DEA certification.

Knowledge, Skill, Ability and Work Difficulty:

  • Able to provide health care in a sensitive, nonjudgmental manner to persons of all economic levels and cultural backgrounds.
  • Capable of independent nursing assessment decisions and nursing management in a primary care setting.
  • Knowledge of medical office practices, procedures and equipment.
  • Skilled in proper English usage, spelling and grammar.
  • Able to recognize the needs and concerns of people within their cultural framework.
  • Able to work independently from general instructions and in the absence of supervision.
  • Skilled at clear and concise oral and written communication.
  • Able to recognize and maintain confidentiality of work materials.
  • Able to interpret Shalom's services and policies to patients and families.
  • Able to self-motivate and be flexible to daily patient schedule and/or clinic needs.
  • Able to understand Shalom's relationships with the community and maintain positive image and relationships.
  • Familiar with Electronic Medical Records (EMR), e-Clinical Works preferable.

Physical Demands and Work Environment:

  • Pass a physical examination after being offered employment and a health assessment annually, thereafter.
  • Clear annual TB, influenza, and other communicable disease testing.
  • Perform under pressure of patients and clinic staff needing assistance to meet deadlines.
  • Reposition a bedbound patient alone, as needed.
  • Exposure to indoor office florescent lights.
  • Exposure to computer monitor.
  • May be required to stand or sit for prolonged time period.
  • May be required to bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.
  • Requires normal hand-eye coordination and dexterity.
  • Requires normal vision capability.
  • Distinguish letter, numbers or symbols.
  • Requires lifting up to 20 pounds frequently and 50 pounds infrequently.
  • Able to perform one person transfer under routine and emergency situations, as needed.
  • May be exposed to hazardous materials.
  • May be exposed to unsanitary conditions in some home settings.
  • May be exposed to high crime areas within the service area community.
