Mazzoni Center, an LBGT Health and Wellbeing agency, has an opening for an HIV Medical Case Manager. Primary responsibility: assess the needs of individuals with HIV disease and assist with linking them to necessary services/resources related to their medical status and psychosocial needs. Provide necessary monitoring, follow-up, and coordination of services, including the provision of treatment adherence. Carry a caseload of 40 clients.
Qualifications: Bachelors degree required in Social Work, Master's preferred or related field. Expertise in mental health/mental retardation, addiction, case management and client advocacy. Understanding of medical and psychosocial issues related to HIV disease and related conditions. Ability to work with diverse populations. Sensitivity to cultural and economic differences. Knowledge of or ability to learn Philadelphia resources and services.
