The qualified candidate will be responsible for the development, implementation and monitoring of the Individualized Service Plans for individuals and their advocates. He/She will also provide case management services to assigned individuals on his/her caseload and determine individual aptitudes, interests and preferences. You will also identify appropriate service providers and link individuals with providers while coordinating services and monitoring individuals satisfaction with the services
Other Job Responsibilities include:
o Conduct intake and screening activities to determine eligibility for services
o Develops, coordinates and monitors Individualized Service Plans (ISP) based upon tenets of 3 I's and P.
o Serves as advocate on behalf of individual and advocate in making decisions regarding choice of service providers, level of service required and program participation.
o Completes billing reports in an accurate and timely manner.
o Maintains individual case notes and file records in accordance with all agency and regulatory requirements, including completion of case notes
o Maintains contacts with residential and program admissions committees in order to advocate for admission to programs and residences
o Maintains contacts with government social services agencies, not-for-profit social service agencies and NYC Board of Education to access resources and/or advocate for services
o Develops a resource database to assist families and consumers in accessing community resources
o Schedules regualr face to face meetings and prepares monthly summary contact notes
o Assists individuals during times of crisis and emergency situations which may occur outside of normal business hours
o Works regularly with, develops, and is responsible for maintaining security of confidential information. Improper disclosure may cause embarrassment, disruption, or conflict to agency or persons receiving services and their families.
Qualifications
Bachelor's degree in Social Work or related Human Services field required
One year experience working with people with developmental disabilities
Bi-lingual (English/Spanish) needed with the ability to read, write, speak and understand English
Good organizational skills along with strong interpersonal skills to effectively interact with employees, co-workers, consumers, families and outside contacts
Able to work flexible hours and travel to diverse neighborhoods
Demonstrated ability through prior experience to provide sound judgment and the capacity to respond to unusual situations
Proficiency in the use of a personal computer
