Description

SCO is looking for 2 well- qualified Direct Care Workers!

They will establish and maintain a positive, healthy climate in which adolescents will be exposed to meaningful life experiences. Assist the adolescent residents in developing and maintaining independent living skills. Assist in the implementation of each resident's service plan, which will enable the resident to work towards the ultimate goal of reunification with family, independent living, or any lesser restrictive setting as determined. Work collaboratively with other staff co-facilitating independent living skills groups and psychosocial rehabilitation groups.

MINIMUM RESPONSIBILITIES:

  • Assists with developing independent living skills, such as budgeting, shopping, laundry and traveling as well as encouraging independence in basic Activities of Daily Living skills such as personal hygiene, menu planning, shopping and cooking
  • Participates in all trainings and required refreshers, as per Office of Mental Health, agency, and program requirements
  • Participates in the provision of mandatory program coverage as needed
  • Assists in house meetings once a week to ensure that all items are covered, including chore assignment, problem solving of peer disputes and recreational planning
  • Participates in admission and orientation of new/perspective residents, and youth admitted for crisis respite
  • Completes all required documentation daily. (Fire Drills, Division of Responsibility sheets, Progress Notes, Billing Logs, etc.)
  • Utilizes agency vehicles to perform various program related tasks and the transport of program residents and/or families to meetings, appointments, home passes, and recreation as required.
  • Assists residents in keeping their scheduled appointments
  • All staff on all shifts (including the overnight shift) must remain awake and alert throughout their shifts
  • Provides immediate intervention to defuse potentially volatile situations that may arise between residents

MINIMUM QUALIFICATIONS:

  • Education/Degree: High School Diploma or GED required.
  • Experience with SED and/or dually diagnosed (SED/MMR) adolescents desirable.
  • Valid and clean NYS Drivers License mandatory.
  • Must obtain acceptable background clearance check through State Central Registry and State Office of Mental Health (finger printing)
