Little Flower Children and Family Services in Wading River, NY is seeking Per Diem Waiver Service Provider's needed for caseloads in Nassau and Suffolk County.
The Bridges to Health (B2H) Waiver Program (located in Hauppauge, NY) is designed specifically for vulnerable children with serious emotional disturbances, children with developmental disabilities, and medically fragile children. The B2H offers services not otherwise available in the community to children with these complex medical conditions, and does so in the context of their often complicated family and caregiver network.
Job Responsibilities include: Attending team meetings to discuss the creation of an Individual Health Plan (IHP) for each participant; Providing services as called for in the IHP with qualified, trained staff; Recording and reporting progress on goals and work with the HCI to routinely update the IHP to reflect current circumstances; Participating in overall quality management for the B2H Waiver Program; May be required to participate in recreation or sporting activities.

- Day Habilitation (for Developmentally Disabled youth only)
- Planned & Crisis Respite
- Immediate Crisis Response
- Crisis Avoidance & Management
- Intensive In-Home Supports
- Vocational Training & Employment Placements for Youth
- Medical Devices/Aids, Assistive Equipment and Accessibility Modifications for the Home and Vehicles
Preferred Qualifications: Master's degree in Social Work or a Master's degree in a Human Services field, plus one year of applicable experience.
Minimum Qualifications: Bachelor's degree, plus two years of related experience.
