Primary Purpose: Educate and recertify qualified clients for Medicaid Managed Long Term Care. Application assistance will require a combination of telephone assessments and in person home visits to assess clients' needs and provide hands on application assistance. Facilitated Enroller will be responsible for tracking client applications, providing follow-up when needed and working with the Program Coordinator to assure all cases are completed within the assigned recertification period.

Qualification:
Ability to work under pressure during deadlines
Strong computer skills (data base, word processing, Excel, etc)
Detail oriented
Education and / or Experience: High School Diploma (required), Background in Public Benefits a plus.
Language Skills: Bilingual preferred; Spanish
Mathematical Skills: Strong. Training will be given.
Physical Demands: Ability to carry 15 lbs and walk up 3 flights of stairs
Work Environment: 80% in the field 20% at Met Council Central.
