Are you a nurse interested in making a difference in the lives of people experiencing homelessness? Then we want you! Pathways to Housing DCis looking for a Registered Nurse to act as a Nurse/Service Coordinator for our Assertive Community Case Management Services (ACT) team. The goal of the ACT team is provide wrap-around services in the community using a team approach. The ACT team consists of social workers, an RN, psychiatrist, addictions counselors, and peer support. The ACT team will employ a "Housing First" model to rapidly reduce homelessness, particularly for those who present with serious mental illness, co-occurring and substance use disorders by providing immediate access to housing without preresquisites.
Responsibilities
The Registered Nurse /Team Leader is responsible for:
Providing direct, flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and co-occurring substance use disorders.
Providing direct supervision to staff.
The RN/Team Leader will spend approximately 50% of their time providing direct services and 50% on administrative tasks.
80% of services will be provided in the community.
Lead responsibility for integrating medical treatment with the tasks of other team members.
Requirements:
RN licensed to practice in theDistrict of Columbia
Management experience
Experience working in homeless services
Experience in behavioral health services
Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations
Career experience in social services, supported housing, or with persons with disabilities
Excellent communication, writing and computer skills
Bilingual English/Spanish a plus
Drivers license needed
