Job Title: Director, Neighborhood Health Transformation

Summary:
Healthy Northeast focuses on the Northeast neighborhood of Hartford, CT, home to over 10,700 people. The initiative organizes residents and over forty public and private organizations to collaborate in improving population health, coaching the most vulnerable residents in managing their health and improving their quality of life while reducing their involvement in costly government-funded systems, and making visible improvements to the physical environment. At its core, it is an effort in capacity building: of residents to identify needs, name solutions, take action and advocate for change; of organizations and government to partner to achieve more efficient and more lasting impact; of newly trained care coordinators and community planners to put their life skills and training to work, partnering with vulnerable people and the community as a whole to improve quality of life.
The Director is an experienced, self-motivated and organized individual and will be responsible for leading and coordinating the initiative through its early years. The Director will work with the strategy Department at central office to link the Northeast Initiative to other CS community initiatives. Particular attention to the social determinants of health will guide this strategic thinking. This position reports to the President, until a Director of Strategic Planning and Business Development is hired. This position supervises a growing team of community-based care coordinators and community organizers.
Essential Duties and Responsibilities:
- Create a cohesive team climate in which accountability for outcomes, use of data-informed decision-making, initiative, good judgment, honest communication and continuous quality improvement are embraced by staff, partners and community residents
- Build consensus across the team on key decisions whenever possible and make concrete decisions to guide the initiative when consensus is not feasible
- Cultivate and manage relationships with state and city government, foundations, corporations, and key partner organizations; oversee funding activities and assist in proposal development
- Manage growing budget, currently at $240,000, to ensure proper stewardship of resources
- Knowledge of Connecticut Medicaid program and ability to leverage partnerships toward sustainable payment
- Oversee the hiring and development of the local team
- Lead the initiative to produce meaningful impacts on population health and to achieve the "Triple Aim" at a neighborhood level: improved health of the neighborhood population, improved experience of the overlapping social care systems, and decreased overall social costs
- Oversee the development of materials detailing Healthy Northeast's core components, processes, activities and outcomes
- Provide stakeholder management, partnership management, resource development and management to deliver successful initiative implementation
- Assist communications staff in project brand management
- Assist strategy team in program planning and refining project-based, community health metrics
- Implement performance measurement and quality improvement for Healthy Northeast in coordination with the strategy team, and based on IHI's Triple Aim initiative and the Rapid Results 100 day framework and management of research and quality assurance protocols
- Interface with external partners to develop promising practices and share learnings
Other Duties:
- Manage partnership with local partners to build community access to healthy foods and to develop an urban farming initiative
- Collaborate closely with colleagues in CS in the "Inspiring Places" unit to ensure successful repurposing of the Swift factory parcel as an engine of Hartford community transformation, and with overall neighborhood planning efforts
Qualifications:
- Master's degree in Public Health, Public Policy, Business Administration, Public Administration, International Development, or a related field;
- Experience with non-profit budgets and finances;
- Experience managing program staff;
- Ability to design effective outcome measurement and process evaluation tools and experience putting measures into practice;
- Fundraising and government relations experience;
- Community Development or Public Health experience;
- Excellent written and communication skills;
- Experience working cross-sector, e.g. economic development and public health;
- Excellent interpersonal skills, including ability to communicate with a diverse set of community members and stakeholders;
- Ability to inspire action among external partners over whom there is no supervisory relationship;
- Strong motivational skills;
- Ability to organize, meet deadlines and delegate appropriately;
- Ability to cope with and embrace change, risk and uncertainty; and
- Willingness to embrace and actively support the unique culture and values of Community Solutions.
Supervisory Responsibilities:
Position is the leader of Healthy Northeast and oversees a growing team that currently includes: Policy and Practice Associate, Community Health Coordinator, and two Community Planning Partners.
Physical Demands:
Requires daily presence in Hartford, with some travel outside of Hartford required.
Decision-Making:
- Ability to exercise excellent independent judgment and ownership of decisions.
- Ability to think strategically, including process level thinking, ability to optimize systems and resources, foresee next steps and set course corrections in place.
- Assist in defining the business and strategic direction of Healthy Northeast in concert with the President, senior management and program directors across the organization.
Financial Responsibilities:
Position is responsible for establishing the annual program budget in concert with the President and Senior Director, Operations and for overseeing the management of program revenues and expenditures against that budget throughout the year. Position is responsible for developing and maintaining relationships with entities that provide funding for Healthy Northeast.
