Job description

ESSENTIAL FUNCTIONS

The Patient Navigator's primary purpose is to perform tasks related to efficient and effective patient flow in the clinic, and assisting NHCLV patients in navigating NHCLV and other services to obtain needed care. Duties include assisting nursing and provider staff with administrative support related to daily patient visits including phone calls, faxing and/or scanning to ensure immediate access to all information required for the patient visit. Following the patient visit, the Patient Navigator makes internal and external appointments and arranges any other services required as a result of the NHCLV visit, including completing referrals and/or pre-authorizations. The Patient Navigator may be required to provide occasional interpreting services for patient visits, and/or occasional back up to Patient Registration. The Patient Navigator must maintain confidentiality of patient information, conduct her/himself in a professional and friendly manner, strictly follow all NHCLV policies, and work cooperatively and supportively with all other staff..

JOB DUTIES

  • Participates in team huddles to review daily patient schedule; follows up as necessary to obtain all lab results, specialist reports, and other materials so that all needed information is available for NHCLV patient visits;
  • Assists provider and nursing staff with daily activities and administrative tasks related to NHCLV patient visits
  • Provide appointments to patients using NHCLC patient scheduling guidelines.
  • Make external patient appointments; completes referrals and/or pre-authorizations with insurance coverage's; coordinates and sends all necessary documentation for external appointments
  • Monitors daily patient schedules and proactively fills vacant appointment slots to ensure all appointment slots are filled; assists with recalls, waiting lists, reminder calls, collection of patient demand, provider availability, and other aspects of optimal schedule management
  • Assist medical patients in accessing dental, pharmacy, WIC and other NHCLV services.
  • Review hospital discharge lists and perform tasks related to appropriate patient hospital follow-up visits, including obtaining all necessary patient information from the hospitals
  • Data collection: assists with accurate and timely capture of patient information; calls patients, hospitals or other agencies to obtain any missing patient information related to billing, grant requirements or other NHCLV reporting activities
  • Performs chart abstraction, scanning and copying as necessary to complete patient visits
  • Performs accurate data entry
  • Follows up on faxing or phone calls regarding patient medication refills or other patient needs
  • Assist with other administrative tasks as assigned by Clinic Operations Manager or designee, including quality improvement processes such as the NHCLV patient satisfaction survey activities.
  • If required, provides back-up to Patient Registration as necessary including checking in patients, verifying insurance coverage, verifying income level for discount fee program, collecting accurate and complete patient information, obtaining copays and other payments from patients; answering NHCLV phones, making appointments as necessary, and transferring calls to appropriate department.
  • Respond to patient requests efficiently.
  • Accurately and legibly fill out all required forms;
  • Provide accurate English and Spanish interpreting for medical staff, as needed.
  • Maintain a courteous attitude toward internal and external customers at all times
  • Other duties as assigned

QUALIFICATIONS

  • High School education or GED
  • Spanish/English bilingual required
  • Prior medical office experience strongly preferred
  • Good verbal and written communication skills
  • Basic math skills
  • Ability to multi-task, work accurately and efficiently in a busy office situation
  • Ability to problem solve, troubleshoot customer issues
  • Computer literate…knowledge of office practices
  • Employee must be able to work hours as scheduled, which may include Saturdays, evenings or extended hours.
  • Ability to work in a team oriented atmosphere
  • Honest, courteous and professional manner
  • Good personal hygiene
