Description

The Doe Fund is currently seeking a self-motivated Case Manager to join our growing team. This position will work with the youth enhancements to the RWA program to better serve existing and incoming youth trainees, ages 18-26. This position will be working out of our Porter Avenue, Brooklyn facility.

RESPONSIBILITIES:

  • Manage a caseload of trainees, monitor their progress toward meeting goals established in their service plans and their participation in required RWA Youth program components.
  • Research and coordinate access to outside referral sources for Mental Health, Substance Recovery, Benefits, Entitlements, and Primary Care services as needed.
  • Collaboratively with trainees, provide assistance in goal setting and self-development; work as a team with other staff to achieve program wide participation, employment, graduation and retention goals among others.
  • Organize and facilitate groups on various topics.
  • Maintain current, accurate, complete and timely documentation of trainee progress in charts and databases (TDF's Client Tracking Database and DHS's CARES).
  • Provide crisis intervention to clients in order to resolve any issues or concerns that may arise.
  • Advocate for appropriate trainee housing and cultivate housing contacts i.e. landlords and real estate offices when necessary for Veteran clients.
  • Interact with Department of Homeless Services and representatives as necessary.

QUALIFICATIONS:

  • Bachelor's Degree
  • At least 2+ years of experience in Human Services/Case Management
  • At least 2+ years working with youth ages 18-26 preferably in a workforce development setting
  • CASAC preferred
  • Strong interpersonal and written/verbal communication skills required
  • Working knowledge of MS, Word, Excel, PowerPoint and MS Outlook
