Family Medicine Physician

Description

Responsible for providing services to all Shalom's patients within the appropriate gender/age range of family medicine services. Family Medicine Physician will work under the supervision of the Medical Director/CMO to assess health priorities and identify needed services:

  1. Provide family medicine services to include primary care and urgent/acute care for all age groups.
  2. Provide access to sub-specialists through on-site services or referrals.
  3. Provide health education and screening to patients.
  4. Review at least 5% of colleagues' charts.
  5. Assists with quality assurance activities: Review charts from colleagues; attend monthly QA/QI Committee meetings; and attend in-services as requested by the Medical Director/CMO.
  6. Maintain understanding of all medical regulations and requirements.
  7. Complete all contract requirements and maintain compliance with established policies and procedures.
  8. Develop medical plan and updates, as needed, for grant applications.
  9. Interpret rules, regulation, and guideline from various funding sources and implements policies and procedures to ensure compliance.

Experience & Qualification Requirements:

  • Experience with serving the medical needs of minority populations.
  • Strong community health/public health orientation.
  • Two years' experience with providing patient care management.
  • Experience working with interdisciplinary teams.
  • Successful or near completion of internship and residency.
  • Knowledge of health systems, preventive medicine concepts, practices, methods and techniques of family medicine.
  • Knowledge of state and federal laws regulating medical profession.
  • Admission privileges at area hospital.
  • Effective oral and written communication skills.

Certifications/Licensures:

  • Medical degree from an accredited medical school.
  • Doctor of Medicine Specialty degree in family medicine.
  • Board certified/eligible Family Medicine Physician.
  • Licensed to practice medicine in the State of Indiana.
  • Board certification or board eligibility in a primary care.
  • Current CPR certification.

Knowledge, Skill, Ability and Work Difficulty:

  • Able to provide health care in a sensitive, nonjudgmental manner to persons of all economic levels and cultural backgrounds.
  • Knowledge of Patient Care Medical Home (PCMH) model.
  • Capable of independent nursing assessment decisions and nursing management in a primary care setting.
  • Knowledge of medical office practices, procedures and equipment.
  • Skilled in proper English usage, spelling and grammar.
  • Able to recognize the needs and concerns of people within their cultural framework.
  • Able to work independently from general instructions and in the absence of supervision.
  • Skilled at clear and concise oral and written communication.
  • Able to recognize and maintain confidentiality of work materials.
  • Able to interpret Shalom's services and policies to patients and families.
  • Able to self-motivate and be flexible to daily patient schedule and/or clinic needs.
  • Able to understand Shalom's relationships with the community and maintain positive image and relationships.

Physical Demands and Work Environment:

  • Pass a physical examination after being offered employment and a health assessment annually, thereafter.
  • Clear annual TB, influenza, and other communicable disease testing.
  • Perform under pressure of patients and clinic staff needing assistance to meet deadlines.
  • Reposition a bedbound patient alone, as needed.
  • Exposure to indoor office florescent lights.
  • Exposure to computer monitor.
  • May be required to stand or sit for prolonged time period.
  • May be required to bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.
  • Requires normal hand-eye coordination and dexterity.
  • Requires normal vision capability.
  • Distinguish letter, numbers or symbols.
  • Requires lifting up to 20 pounds frequently and 50 pounds infrequently.
  • Able to perform one person transfer under routine and emergency situations, as needed.
  • May be exposed to hazardous materials.
  • May be exposed to unsanitary conditions in some home settings.
  • May be exposed to high crime areas within the service area community.
