We are currently seeking qualified candidates for the Bilingual IBCCP Patient Navigator. This role provides access and support services for uninsured women and transgender patients receiving breast/chest and cervical cancer screenings.

Principal Duties and Responsibilities:

  • Coordinate medical care with providers, schedule patient follow-up (internal and external appointments) as instructed by providers to ensure timely delivery of diagnostic and treatment services, as it relates to uninsured women and transgender patients receiving care through the Illinois Breast and Cervical Cancer Program (IBCCP) and Stand Against Cancer subcontract.
  • Facilitate appointments with collaborative medical partners for provider-recommended follow-up screenings and treatment, ensuring that appropriate medical records are available at scheduled appointments.
  • Maintain communication with patients, families of choice, and health care providers to monitor patient satisfaction with the experience.
  • Liaison with appropriate staff, including finance, medical providers and billing coordinator to maintain appropriate and ongoing billing of services, including Howard Brown Health Center billing of third party collaborative partners.
  • Document all interactions with patients as necessary to ensure positive outcomes.
  • Serve as agency representative at IBCCP-related meetings, coalitions, conferences, phone calls, and email communications.
  • Manage data entry work in Cornerstone Database including records and reports.
  • Participate in Quality Assurance process via data collection, evaluation of results and implementation of recommendations.
  • Ensure grant compliance related to patient data, as well as monthly and quarterly outcomes reporting.
  • Work with staff members of the Lesbian Community Care Project (LCCP), provide input on outreach and programming opportunities, as well as support of staff, volunteers and interns related to LCCP.
  • Attend all department meetings.
  • Participate and represent interagency and stakeholder meetings as needed. Develop and maintain ongoing relationships with other service providers.
  • Adhere to Howard Brown Human Resources Policies and Procedures.
  • Adhere to OSHA standards in delivery of care to all patients.
  • Maintain patient confidentiality at all times.
  • Perform other duties as assigned.

Knowledge, Skills and Abilities:

Required:

  • Licensure (LSW, LCSW, BSN, RN) with experience in a medical setting.
  • Fluency in Spanish.

Preferred:

  • Experience with women's health nursing and/or public/community health.
  • Knowledge of and experience with multi-ethnic, gay, lesbian, bisexual, queer, transgender clients, as well as issues related to breast and cervical cancer.
  • Experience with grant/project management.
