Summary
Coordinates, facilitates, and provides case management services to program participants. Duties include providing an ongoing support and expertise through a comprehensive case management assessment, planning, implementation and overall evaluation of program participant needs.
Primary Responsibilities
- Responsible for providing a comprehensive case management services to program participants, including intake assessment, short and long-term case plan development. Assists program participants with crisis interventions.
Specific duties include, but are not limited to:
- Scheduling and facilitates one-on-one meetings with program participants to assess their needs and create Individual Service Plan (ISP).
- Conducts group psycho educational group, including, but not limited to, harm reduction, parenting, budgeting, and anger management.
- Provides referrals for other organization providing program participants with information and referrals for other service providers offering services such as housing, financial, education, medical, counseling, or other areas of need.
- Monitoring all progress reports pertaining to program participant's ISP throughout enrollment and at a minimum of twelve (12) consecutive months immediately following participant's exit date. Meets weekly with the Associate Director of Training to report participant's ISP case status.
- Maintains program participant's case file.
Specific duties include:
- Collecting basic information from participants to begin a case file
- Verifying, updating, and entering participants information into the case file
- Responsible for documenting all program interaction with participants.
Specific duties include, but not limited to:
- Writing daily case notes
- Submitting weekly case notes and reports to the Associate Director of Training.
- At the direction of the Associate Director of Training, creates and submits case notes and reports in accordance with grant requirements as needed and appropriate.
- Advocates for program participants as needed and provides follow-up on all program participants.
- Responsible for implementing clinical protocols/policies for the program.
- Works in conjunction with the Associate Director of Training to supervise and evaluate BSW students.
Secondary Responsibilities:
- Works with the Program Director and Associate Director of Training to ensure partipant feedback is included in planning for future outreach and program growth.
- Conducts periodic reviews and audits of records to ensure an active, accurate program information system based on proper service control/standards, and regulatory agencies.
- Participates in staff and team meetings. Communicates and coordinates with Associate Director and appropriate staff as needed.
- Performs other related duties as assigned by the Associate Director of Training.
EDUCATION AND EXPERIENCE:
- MSW Degree from an accredited University or College.
- Must have a minimum of one (1) year case management experience.
- Strong computer and note-taking skills required. Experience with Microsoft word and excel programs required.
- Must possess knowledge with community resources and referral process, particularly in Baltimore City.
- Experience with working in a non-profit organization strongly preferred.

- Must have the skill and ability to establish and maintain effective working relationships with program participants.
