At APEX Hospice and Palliative Care, our mission is to provide comfort and compassionate care for individuals and their families who are living with terminal illness. Our care promotes patient autonomy and encompasses individualized quality care for the mind, body, and spirit. We strive to ease suffering and maintain the dignity of our patients by providing superior quality care. It is the goal of APEX Hospice and Palliative Care to maintain the highest ethical and moral standards and exhibit the utmost professionalism, compassion and concern towards patients, families, and fellow employees.

We understand that the quality of life is valued by our patients and their families and we strive to develop lasting partnerships with them to make the Hospice experience special and memorable. We care for people with all end-stage diagnoses, including: Alzheimer's, Pulmonary Disease, CHS, Cancer, Liver Disease, Dementia, Parkinson's, Renal Disease, Hunington's Disease and ALS.

APEX Hospice provides individualized hospice services wherever our patients and families call home. This may include a personal residence, nursing facility, assisted living residence or a hospital. The care provided by us does not replace those services provided by the assisted living or nursing home staff. We are an additional support system with individualized care provided by our Interdisciplinary Hospice Team; we communicate regularly with the Nursing Facility to ensure we work together to meet your loved one's care, needs and goals for compassionate quality of life.

THE GOAL OF HOSPICE CARE IS TO HELP A PERSON'S LAST MONTHS BE AS FULFILLING AND COMFORTABLE AS POSSIBLE.

Hospices depend on volunteers, without the effort of volunteers, hospices could not continue their important work. Volunteers are the backbone of the hospice team. They form close bonds with patients and family members, help people communicate and grieve with the family.
