What is HealthCorps?
Community HealthCorps (a branch of AmeriCorps) is intended to promote health care for America's underserved populations, and helps to develop tomorrow's health-care workforce. Community HealthCorps navigators provide direct service to the clients at Sea Mar Community Health Centers while enhancing their skills and experiences in the field of health and human services. Each member's personal growth is fostered through a highly supportive, challenging and educational team environment.
The HealthCorps positions are all 11-month commitments, beginning August 19th, 2013, and lasting through July 3rd, 2014. All positions are full-time commitments, with a total of 1700 service hours required by the end of the service term.
Patient Navigator Position:
Language Requirement: Fluency in at least one of the following languages: Mandarin, Cantonese, and Vietnamese.
The ICHS Patient Navigators will work with clients at two International Community Health Services Clinics, one in the International District and one in Holly Park. ICHS as a whole specializes in service to medically underserved Asian immigrant communities and offers a traditional Chinese medicine clinic. The Patient Navigators will work with various health service providers to assist patients in accessing appropriate care, establishing a medical home, ensuring their pre- and post-visit needs are met and facilitating brief health education encounters. The navigators will also provide translation/interpretation services and help coordinate the Reach Out and Read Program located at the clinics.

Eligibility:
To be eligible for this program, an applicant must be at least 18 years of age with a high school diploma. Applicant must be a U.S. citizen, US national or permanent legal resident at the time of application. Applicant must also possess a strong interest in serving the community within the health and human services fields.
