HealthCorps Patient Navigator and Renacer Health Educator- Seattle

What is HealthCorps?

Community HealthCorps (a branch of AmeriCorps) is intended to promote health care for America's underserved populations, and helps to develop tomorrow's health-care workforce. Community HealthCorps navigators provide direct service to the clients at Sea Mar Community Health Centers while enhancing their skills and experiences in the field of health and human services. Each member's personal growth is fostered through a highly supportive, challenging and educational team environment.

The HealthCorps positions are all 11-month commitments, beginning August 19th, 2013, and lasting through July 3rd, 2014. All positions are full-time commitments, with a total of 1700 service hours required by the end of the service term.

Patient Navigator and Renacer Health Educator Position:

Language Requirement: Bilingual Spanish/English required

The navigator will split their time between the Seattle Medical Clinic and Renacer, Sea Mar's in-patient youth treatment center. As a patient navigator at Seattle Medical, the navigator will work with various health service providers to assist patients in accessing appropriate care, establishing a medical home, ensuring their pre- and post-visit needs are met and facilitating brief health education encounters. Navigator will provide translation/interpretation services and coordinate the charity drug assistance program.

In addition to this, the navigator will provide health education classes and opportunities to the young men at Renacer. They will work alongside the full time health education navigator there to coordinate and facilitate various classes around topics such as nutrition, sexual health, mental health, and family planning.

Eligibility:

To be eligible for this program, an applicant must be at least 18 years of age with a high school diploma. Applicant must be a U.S. citizen, US national or permanent legal resident at the time of application. Applicant must also possess a strong interest in serving the community within the health and human services fields.
