This position requires you work as an independent Social Worker.
You will provide social work services to adult inpatients and outpatients diagnosed with cancer and other illnesses. Provide psychosocial assessments, individual, family and small group therapy, supportive counseling and grief and bereavement counseling. Provide strategic and medical crisis counseling for patients and families in coordination with the primary medical team and adjunct consulting services. Coordinate and implement discharge plans by arranging for transition to the next level of medical care in compliance with the multidisciplinary treatment plan. Function as a member of various medical surgical teams, presenting reports at team conferences and participating in evaluation and treatment planning for patients. Plan, coordinate, and provide comprehensive referral services to patients and families. Provide in-service training about complex social work services to multi-disciplinary treatment teams. Provide training for paraprofessionals including: social work students, social work interns, social work fellows, and mental health specialists.
You will liaise with the ministry of health, social reformation centers, schools and other stakeholders to carry out outreach activities when necessary.
