Position Overview 

CHW Cares Health Coaches are hired from the communities that we serve and are trained by CHW Cares to provide individual coaching sessions to help patients implement realistic, culturally appropriate lifestyle changes to improve their chronic conditions. With shared life experience, our Health Coaches have a unique ability to motivate health behavior change through individualized coaching and ensuring each patient stays on track with their care journey. Our Health Coaches also help connect patients to valuable resources to get support with non-medical needs.

·      Role: CHW Cares Health Coaches meet patients at the CHW Cares medical clinic in Washington Heights, in the home, or convenient community settings to help our patients better manage their health. Health Coaches work as part of a primary care team within the CHW Cares primary care department and receive direct supervision from a Chronic Care Supervisor (Registered Dietitian).  The job entails three components:

o   Outreach and enrollment: Each Health Coach is responsible for enrolling patients into his/her panel. After reaching a full panel, further enrollment is periodic to fill in gaps when a patient moves on. Enrollment is achieved through a combination of securing referrals from our own physicians, conducting clinic-based outreach activities (i.e. tabling), and conducting outreach in community settings.  

o   Educational Health Coaching: After enrolling, Health Coaches meet with each patient to teach them the skills, knowledge, and confidence to manage their chronic conditions on a day-to-day basis. Examples of skills include nutritional management of diabetes on a budget, correct use of an asthma inhaler, and how to monitor blood sugar levels on a daily basis.   

o   Ongoing support:  Health Coaches continue to follow-up with each client on a regular basis to provide concierge level support to ensure they stay on track with their care journey. For example, Health Coaches ensure patients understand how to schedule appointments. When a patient has an urgent medical need, Health Coaches are the first to assess the need and escalate the need to the right member of the CHW Cares primary care team.

·      Training: No prior medical experience is required to become a CHW Cares Health Coach. CHW Cares prepares candidates to become Health Coaches through a full-time, paid training period, which is followed by continual learning support & dedicated clinical supervision.  

o   Graduation requirement: In order to retain this role beyond the training period, successful completion of full-time, paid training period with a comprehensive graduation exam is required to transition from Training to ongoing employment

·      Geographic Requirement: Live in or be familiar with the communities that we serve. The current opportunity is open to job candidates from the Bronx and Washington Heights/Inwood neighborhoods of upper Manhattan.

Responsibilities

·      Educate, motivate and support individuals living with uncontrolled chronic illness in ways to achieve realistic, culturally appropriate lifestyle changes to help self-manage their illnesses. 

·      Maintain high standards of productivity and quality with regards to client engagement and retention.

·      Communicate health issues to Chronic Care Supervisor in a timely way, as necessary.

·      Conduct individual case reviews and problem solving with Chronic Care Supervisor.

·      Provide regular feedback and troubleshoot with supervisors and fellow health coaches to develop solutions that improve members’ health and operational workflows.

·      Engage in collaborative care meetings with an interdisciplinary care team that includes Physicians, Dietitians, Care representatives, and more.

·      During periods focused on recruitment, conduct outreach tasks to increase referrals.

·      Demonstrate willingness to take on new assignments, learn new things and adapt to technological improvements and organizational change.

·      Demonstrate proficiency in planning, organizing and prioritizing workload using appropriate technology (i.e. phone, tablet, email, etc.).

·      Complete tasks including session documentation, coaching and health education accurately and in accordance with organizational guidelines.

·      Serve as a positive representative of the organization internally and externally.

·      Demonstrate cultural competence, intellectual curiosity, and desire for continual learning.

·      Ensure compliance with all organizational privacy, security, and compliance procedures. Communicate concerns or questions to the Privacy, Security, and/or Compliance Officer(s) when necessary in a timely way.

Qualifications

·      Minimum of High School diploma or GED.

·      Proficient in use of technology (smart phones, tablets, computers)

·      Must be able to manage a caseload and prioritize activities.

·      Must possess exceptional interpersonal and organizational skills.

·      Keen active listening and motivational skills.

·      Good judgement and character.

·      Committed to continual learning & to problem-solving.

·      Works well independently and in teams. Respects unique needs and diverse backgrounds of members of the community.

·      Work authorization for employment in the U.S. is required.

·      Demonstrated personal commitment to evidence-based medical guidelines, healthy living and wellness.

·      Bi-lingual Spanish language ability required
