HTC providers will do the HIV screening test and counselling. Then HIV positive clients will be given a follow-up form (See attached form) and be referred to a health center for continual medical care. Qualified HTC providers will collect and present the forms to certified medical doctors. After the clients are connected to their preferred health centers, we will collect HIV/AIDS related information from those health centers with the consent of our clients. We shall monitor the progression of HIV/AIDS of our clients and make individual counselling.
